Allegheny Recovery Partnership Plan

The Allegheny Recovery Partnership Plan outlines an evidence-based approach to building an improved and interconnected SUD treatment system. The plan will expand step-down care for people who can receive appropriate treatment at a lower-level of care and will broaden access to recovery housing in which people can continue outpatient treatment in a safe and supportive environment. By reaching more people in these settings and reducing reliance on repeated residential and crisis-level care, the County expects to generate savings that it can reinvest in assertive outreach and retention (AOR) programs. These programs will focus on continuity of care and will respond to changing needs before they escalate to crises that can intensify disruptions in individuals’ lives and carry significant costs for the systems that serve them.

The plan centers on three overlapping improvements to the SUD treatment system:

• Enhancing SUD residential treatment (colloquially known as “rehab”) through a series of new contracts with a select group of providers committed to increased coordination of care with outpatient treatment providers.
• Creating and financing an assertive outreach and retention (AOR) intervention that will work with patients in the community to keep them engaged in care, re-engage individuals who have fallen through the gaps between services, and support those who are cycling through crisis levels of care (e.g., hospitals, emergency departments, jails, residential levels of treatment).
• Creating a unified front door to treatment to enhance coordination for individuals seeking SUD treatment or transitioning across treatment episodes or services.
